Stan blew out his right knee during a college track meet, an injury which effectively ended his future career as an athlete. It's this injury that leads him to take drastic measures and agree to try an experimental serum, which ends up giving him super-speed.
His speedster powers do cover some basic preventative concerns (e.g., he can run at speeds which would probably flay a normal human and be fine), but he's still susceptible to injuries such as road rash or abrasions if he wipes out while running at any speed, and while his speed doesn't heighten it, he still deals with the foot aches and pains of any runner.
